Hello,

in function url_status you use the regex:

if (preg_match("/HTTP/[0-9.]+ (([0-9])[0-9]{2})/i", $answer, $regs)) {

it shoud be

if (preg_match("/HTTP\/[0-9.]+ (([0-9])[0-9]{2})/i", $answer, $regs)) {


You forgot to escape the / in the middle, else it's not working properly and returns state ok on 404 errors.
